Once you have designed and developed your blended course, the next steps are delivery and evaluation.  The ensure useful feedback is gathered to support  iterative design processes, peer review is often used.  This form, developed by the University of Central Florida, has been developed to guide that feedback process.  It is broken up into four parts:
